The purpose of the questionnaire

Services for patients with prostate cancer are undergoing reform to ensure that effective care is delivered in a way that meets the preferences of the patients themselves. The aim of these questionnaires is to allow services to be reviewed so that health care professionals can see the extent to which they are meeting the needs of patients and their carers.

If two answers have been given when only one answer was required please enter 99.
